Contact Information

Fill in all the required information. If you are unsure of your congregation I.D.# or what district your church is part of, click the help link next to those fields.

If you wish to participate in the Rally Band or Choir, fill in all information in the appropriate section.
If you do not wish to sign up for either, choose the appropriate circle and leave all additional band/choir fields blank.
If you are supposed to be chaperoning/monitoring youth from your group who are not in band or choir during rehearsal times and are unable to find another leader from your group to cover for you, DO NOT sign up to be in the band or choir.

EACH LEADER AND YOUTH MUST HAVE THEIR OWN UNIQUE E-MAIL ADDRESS.
Select the number of female youth leaders (if you are female, include yourself in this number). Fill in each leader’s name and the female youth she will responsible for (including their e-mail addresses). If you are the Contact Youth Leader, check the appropriate box under your name on the female page.
A maximum of six females can be assigned to each leader. You may assign less than six if you wish. You need at least one leader, 20 years of age or older, for every six females. Example: if you have eight female youth, you will need at least two female leaders 20 or older.

Female youth will be in roommate pairs (two females per room) according to the colored pairs on the form (i.e., make sure that roommates are in the same color). "Leftover" female youth will be paired with another "leftover" female youth from another congregation. Read through the Christian Conduct Covenant. Please check the box located beneath the Covenant. You cannot advance the form without agreeing to this.

The total number of youth and leaders you’ve registered (NOT including yourself) will automatically total for you. At the bottom of the screen you will see the total amount you owe ($295 for yourself, $50 for each youth and youth leader).

Enter your billing information as it is on file with your credit card issuer or bank. If it is the same as your previously entered contact information, you may click "prefill your address below" and it will automatically fill in the corresponding fields for you.
Choose payment method by either credit card or electronic check. Fill in all the approriate information.

After clicking "Submit," review all the information you have entered. If any of it is incorrect, click your browser's "Back" button and correct or re-enter the information.

If all the information is correct, click "Checkout." If your payment is accepted, a "thank-you" screen will appear and the registration information has been transferred to the CYD office. You will receive a confirmation e-mail as well.
If your payment is denied, you will receive a processing error. Your registration will NOT be transferred to the CYD office until payment is accepted. Common problems with denied payments are: billing information was incorrectly entered/did not match what the bank has on file, your credit card or checking account has a daily spending limit that was exceeded, your card is expired, the amount was large enough that your card's issuing bank tagged it as possibly fraudulent (it's not a bad idea to call your card's issuing bank to let them know they should expect a large transaction to be attempted with WELS), or other issues/errors.
All your leaders and youth will also receive an e-mail with a link that will allow them to complete their registration anytime before May 1st, 2009.
